The Rule Behind It

What OSHA Asks For, Paragraph by Paragraph.

The tool follows these rules. Read the full text on eCFR before you rely on it: this is decision support, not legal advice.

29 CFR 1904.4(a)
Record each fatality, injury and illness that is work-related, is a new case, and meets one or more of the general recording criteria in 1904.7 or the special criteria in 1904.8 to 1904.11.
29 CFR 1904.5(a)
A case is work-related if an event or exposure in the work environment caused or contributed to it, or significantly aggravated a pre-existing condition. It is presumed, unless an exception in 1904.5(b)(2) applies.
29 CFR 1904.6(a)
It is a new case unless the person already had a recorded case of the same type on the same body part and had not fully recovered.
29 CFR 1904.7(b)(1)
The general criteria: death, days away from work, restricted work or job transfer, medical treatment beyond first aid, loss of consciousness, or a significant diagnosed injury or illness.
29 CFR 1904.7(b)(5)(ii)
First aid is a closed list of 14 treatments. Anything not on it is medical treatment.
29 CFR 1904.8
Record needlesticks and sharps cuts contaminated with another person's blood or other potentially infectious material.
29 CFR 1904.9
Record a medical removal under an OSHA standard's medical surveillance rules.
29 CFR 1904.10
Record a work-related Standard Threshold Shift when the total hearing level is also 25 dB or more.
29 CFR 1904.11
Record a TB infection after a workplace exposure to a known case of active tuberculosis.
29 CFR 1904.29(b)(3)
Enter a recordable case on the 300 log and the 301 within 7 calendar days of learning about it.

Questions About the Is It Recordable? OSHA Recordability Checker.

Have one that is not here? Ask X3, or write to us and a person will answer.

Is a doctor visit recordable?
Not by itself. A visit only for observation or counseling, or for tests like an x-ray, is not medical treatment. What matters is the treatment given, days away, restriction and the other results in 1904.7.
Is a prescription recordable?
Yes. A prescription medicine, or a non-prescription medicine recommended at prescription strength, is medical treatment beyond first aid.
Are stitches recordable?
Yes. Stitches, staples and skin glue are medical treatment. Butterfly bandages and Steri-Strips are first aid.
What about a temp worker?
If you supervise the temp day to day, the case goes on your log, not the staffing agency's (1904.31).
Is this the final answer?
It is decision support. A qualified person at your site decides, and should check the full text of 29 CFR 1904 for unusual cases.
Is my answer sent anywhere?
No. The checker runs in your browser and keeps nothing on our servers.
